  Q: What is home health care?      
  A: Home health care is care provided to individuals and families in their place of residence for promoting, maintaining, or restoring health; or for minimizing the effects of disability and illness. Home health care refers to home visits by professionals including nurses, doctors, therapists, social workers, and home health aides.  
         
  Q: Who is eligible?      
  A: The patient must be essentially homebound and may not be bedridden but must require assistance to leave his/her place of residence.  
       
  Q: Who will pay for my services?      
  A: Express Professionals Home Health Services, Inc. will collaborate with Medicare and Private Insurance to provide appropriate services to meet patient’s coverage and needs.  
         
  Q: How can a patient start receiving home health care?  
  A: The patient must be under the care of a physician, the physician’s plan of treatment will include orders for all skilled services. Upon receipt of a referral, an evaluation by Registered Nurse and or appropriate Therapist will be completed to determine that care can be adequately and safely performed in the patient’s place of residence, and to assess patient’s care needs, also to ensure that the patient meets the admission criteria.
